NORFOLK, Neb. -- One person is dead after an apparent workplace accident in northeast Nebraska. 

The Madison County Sheriff's Office says deputies and rescue crews responded to the Continental plant outside of Norfolk on Saturday evening. 

A man had fallen from a lift and hit his head. 

The man, identified as 50-year-old Keith Polenske of Randolph, was taken to Faith Regional where he later died from his injuries. 

Authorities say witnesses saw Polenske working on a scissor-lift, when an automated crane struck the lift and caused it to tip over. 

The investigation is being handled by the Madison County Sheriff's Office and Madison County Attorney Joe Smith.
